There is new (?) category of extreme by area and that is the most central cache. 

I know how to find all other types by any country/county but here i don't know where to find it; don't know any easy way nor any long and painful one

Also came here with this struggle.  There has to be a filter available somewhere for this if Project-GC can calculate and show the flag for it.  I wonder if it just hasn't been made available to us yet?

I found the most central point in Missouri (Chat GPT) and then used those coordinates to search for the nearest cache.  I'm planning to go and find it and see if the flag in my state activates on my profile stats from PGC.

There isn't really a good way to find these as there's currently no tool for them.

My suggestion for a "long and painful way" to find them would be to use the northern, southern, western and eastern caches to make a intersection of lines calculation (for example with geocachingtoolbox) and then check the profile stats of people that have found caches in that area for a cacher that have logged the most central cache.
